Picture Order Forms

Picture Order Forms will be available at check-in.
Forms should be filled out in advance or in the dressing room.
Have your forms filled out completely prior to the class shoot.
Payment must be received in full with the first form.
On the order form clearly list:
Child's Name/ Telephone/ Email

There is no fee to participate in class group shots.
You will notice that Gary Antle Productions has graciously discounted all packages. 

All forms must be directly handed to the teacher by either the dancer and or parent of the child. This is how we determine which children are having individual pictures taken. Dancer's without a form to present at the time of the session will not have an individual taken.

Please be respectful of our photographer. This will be a closed shoot. Parent/guardian will be asked to remain in the dressing room or in the school lobby. Only dancers, photographer and faculty will be permitted in the designated photo area.
